Compounded medications are not approved by the U.S. Food and Drug Administration (FDA). The FDA does not review compounded medications for safety, effectiveness, or quality before they are marketed in the same manner as FDA-approved medications. Compounded medications may differ from commercially available FDA-approved medications in formulation, dosage strength, dosage form, inactive ingredients, appearance, packaging, storage requirements, beyond-use dating, or method of administration. A pharmacy's state licensure does not constitute FDA approval of its compounded medications. 4) Prescription & Fulfillment Process All prescription medications require an evaluation and prescription by an appropriately licensed healthcare provider. The prescribing provider independently determines: Whether treatment is medically appropriate; Whether a compounded medication is clinically indicated; The medication prescribed; The formulation, strength, dosage, and duration of therapy. ReviveRX Pharmacy is independently responsible for: Reviewing and processing prescriptions; Compounding and dispensing medications; Confirming authority to dispense to the patient's state; Labeling and packaging medications; Providing required pharmacy instructions; Shipping medications; and Maintaining all required pharmacy licenses and regulatory compliance. Telegenix does not manufacture, compound, package, label, or dispense prescription medications. 5) Pharmacy Relationship Contractual Relationship Telegenix maintains a contractual and/or administrative relationship with ReviveRX Pharmacy to facilitate prescription fulfillment. These arrangements do not influence the independent clinical judgment of the prescribing healthcare provider or the professional judgment of the dispensing pharmacist. 6) Medication Safety Patients should use prescription medications only as directed by their prescribing healthcare provider and according to the instructions provided by ReviveRX Pharmacy. Patients should carefully review the pharmacy label and accompanying medication instructions with each prescription they receive. Questions regarding medication preparation, ingredients, packaging, storage, shipping, or product quality should be directed to ReviveRX Pharmacy. Questions regarding treatment, dosage, side effects, or medical suitability should be directed to the prescribing healthcare provider. Patients experiencing a medical emergency should call 911 or seek immediate emergency medical attention. Do not use medication if the packaging appears damaged, the seal is broken, or the medication appears contaminated or materially different from its description.
